Story Reveal Ledger

Tracks what each story entry is allowed to expose, what it actually exposes to the player, and which authority files or gate conditions justify that reveal. Use this ledger to keep story artifacts below the exposure ceiling defined by the cited lh-*, ul-*, ud-*, or caution-surface files.
